Intake Form Requirement:

The intake form must be completed as part of onboarding for BOTH individuals. It can be completed before or after purchase, but it is REQUIRED to begin your files.

No information = no results.
If the intake form is not completed, your service cannot be started.

Credit Monitoring Requirement:

Each individual must have an active credit monitoring account.

The direct sign-up link will be provided inside your onboarding file after purchase.

This includes a 7-day $1 trial, followed by $24.99/month per person, and must remain active throughout the program.
